What "Human-in-the-Loop" Actually Means in Production

Human-in-the-loop is not a buzzword you slap on a slide to reassure legal. In production, it means designing explicit checkpoints where a human reviews, approves, or overrides an AI action before it affects a customer, a transaction, or a compliance record.

The art is knowing where checkpoints belong and where they slow you down. High-stakes, irreversible actions — refunds above a threshold, account closures, regulatory filings — need gates. Low-stakes, easily reversible actions — draft suggestions, internal summaries — should flow without friction.

We typically design three tiers: auto-approve for low-risk actions, queue-for-review for medium-risk, and hard-block for anything that requires a named approver. The tiers are configurable per workflow, not global.
